The state has fined and cited a Verona assisted living facility over the death of a 93-year-old Alzheimer's patient who died of hypothermia outside the facility, the State Journal reports.


Four Winds Lodge failed to check on the resident, a state inspection report says, and didn't put an alarm bracelet on Edna Biederman even though she was seen displaying "exit-seeking behavior" three days before she died on March 8, state Department of Health Services inspectors said.


The state fined Four Winds $845 and gave the facility a citation for failing to provide adequate supervision.
